Which Type of Implant is Right for You?

Dr. Ennis offers his clients three types of breast implant options so that they can choose which one is right for their body and lifestyle. During your consultation with Dr. Ennis, he will discuss these options with you further so that he can help you achieve the look and feel that you desire.

Types of Breast Implants

Saline Implants

Saline implants consist of a silicone outer shell and are filled with a sterile saline solution.

The advantages associated with these implants are:

  • Filled with sterile salt water
  • Most economical implant choice
  • Size-adjustable implant (filled after insertion)

Some disadvantages of saline implants are:

  • Less natural looking and feeling than other implant options
  • More prone to folding or rippling
  • May be visible, and you may be able to feel the implant, underneath the skin surface for patients with thin skin or minimal body fat

Silicone Implants

Silicone implants are made up of an outer, silicone shell filled with a cohesive silicone gel filling. Although these are not the form-stable implants, they are often referred to as the “gummy bear” implants.

Some advantages of these types of implants are:

  • Similar to the look and feel of natural breasts
  • Wrinkles and ripples are much less likely than with saline
  • Generally lasts longer than saline implants

Some disadvantages of silicone implants are:

  • More expensive than saline implants
  • Not as easily detectable if it breaks or leaks

Form-Stable Silicone Implants

The newest generation in silicone implants, the form-stable, or true “gummy bear,” implant has an outer silicone shell filled with a form-stable silicone filling that maintains its shape. This implant is also known as the anatomic, or tear drop shaped, implant. The outside of the implant has to be textured to prevent the implant from rotating in the breast pocket.

Some advantages of form-stable silicone implants:

  • Often used for breast reconstruction
  • Stable filling maintains shape and position, avoiding rippling and folding
  • Available in a range of shapes and sizes
  • Less likely to break or leak than other implants

Some disadvantages of form-stable silicone implants:

  • Placement requires a longer incision than other implants
  • More expensive than other implant options
  • Will not move like a normal breast or smooth implant, so it is more difficult to push them up in a bra, or together, to achieve cleavage

Dr. Ennis Explains Breast Implant Sizes

Which Incision Point Fits Your Needs?

Another decision you will have to make when planning your breast augmentation surgery is where to place the incision through which the implant will be inserted. Dr. Ennis uses three incision points, and will discuss which may be suitable for your surgery during your consultation.

Transaxillary Incision

This is the incision point preferred by Dr. Ennis and most of his patients, as it is less invasive than other incisions and leaves no scar on the breast. The doctor makes a small incision in the crease of the armpit, leaving a small, almost invisible scar hidden in the arm fold, which gradually fades away into the arm crease after a couple of months.

Both saline and silicone implants can be placed using this incision point in an endoscopic breast augmentation surgery, where a long, thin instrument mounted with a small camera called an endoscope is inserted in the incision to visually guide the doctor as he works. This allows the doctor to see everything on a high-definition television so that he can precisely make the pocket and place the implant.

Oftentimes, a Keller Funnel™ is used to assist in the placement of the silicone implant through the underarm. This allows even a large silicone implant to be placed through a small incision in the underarm.

The transaxillary incision and approach is highly technical, and other surgeons may give you many excuses as to why implants should not be placed this way. Often, this just means that they are not comfortable with the endoscopic approach and the advanced technology necessary to achieve the outstanding result through the underarm.

Types of Breast Augmentation Incisions

They may choose one of the other incision options, as they are not concerned about leaving a scar on a woman’s breast.

Periareolar Incision

This incision around the areola is made in the dark-colored skin around the nipple. But because this incision involves possible interference with nerves and milk ducts, it may lead to loss in nipple sensation and difficulty breastfeeding.

Inframammary incision

The inframammary incision is made under the breast, in the breast fold. As a result, the scar is well-hidden by the breast and by a bra or bikini top if the patient has a natural fold to their breast (though it may be visible when the arms are raised and the breasts lift).

The scar, though less concealed than the underarm scar, will most likely fade over time and with proper care.

As such, it may be an ideal option for women who would like to prevent breastfeed complications and possible nipple sensation loss, both of which are risks with the periareolar incision. For many complex revision procedures, this may be the only option to be able to correct concerns from previous surgeries. This scar will fade nicely over the course of a year.

Where To Place Your Implants?

Along with the type of implant and the incision location, you have a choice of where the implant will be placed. Dr. Ennis will further explain these placement options with you during your personal consultation, where he can suggest an option that best suits your build as well as your surgery goals.

Submuscular Placement

This option places the implant underneath the pectoral muscle.

Subglandular Placement

This option places the implant over the pectoral muscle and breast tissue.

Dual Plane Position Placement

This option places the implant partially over the pectoral muscle (but underneath the breast tissue) and partially under the pectoral muscle.

In almost every situation, the implant should be placed at least partially under the muscle. When the implant is placed under the muscle, or at least partially under the muscle, the shape of the breast is more natural, a round implant takes on a more tear-drop shape, there is less interference during mammograms and the capsular contracture risk is significantly lower!

Breast Augmentation Procedure Overview

Breast augmentation surgery is an outpatient procedure that is usually performed at an accredited hospital, a surgery center or an accredited office-based surgery suite. The entire procedure usually takes less than one hour, during which time the patient will be placed under a light general anesthesia to ensure their maximum comfort during surgery.

Dr. Ennis begins the surgery by making an incision — in the areola, breast fold or armpit — and placing the implant through the opening. Saline implants are placed empty and filled once in place.

For the transaxillary incision (which is Dr. Ennis’ preference, as it leaves no scar on the breast), an endoscopic breast augmentation is performed with the use of a small camera attached to a long, thin instrument (or “endoscope”) that is inserted through the armpit incision and which allows the doctor greater visibility during surgery. The entire surgery is performed on a high-definition television screen, which allows for very accurate placement of the implant.

Once the implants are in place and properly positioned behind the nipple, Dr. Ennis will close the incision with a dissolvable suture and cover the breasts with a waterproof dressing that will allow the patient to shower the next day. A soft stretchy band is worn over the top of the breast to help hold them in position for the first week. Someone will need to drive the patient home and stay with them for the first 24 hours after surgery.

Breast Augmentation Recovery

The first day after surgery, Dr. Ennis will give you instructions on some light exercises and massage to perform during this time, including raising your arms over your head. You should also be up and walking around, even the first day, to help with the swelling. Avoid lifting, pushing or pulling movements for the first two weeks.

Dr. Ennis does not recommend taking aspirin or ibuprofen for the first week, as these medications may increase the risk of bleeding.

Dr. Ennis will schedule follow-up appointments, starting the first five to seven days after surgery, so that he can check your healing progress. Patients are usually able to return to non-physical work (like a desk job) after five to seven days, and after seven to 10 days for more strenuous jobs (like bartending or cutting hair).

Most people are back to doing light exercises the second week, and back to running after two weeks. Chest exercises, such as bench pressing and push-ups, should not be resumed until at least six weeks after surgery.
