Breast Revision Surgeon – Correcting Implant Problems

Dr. Ennis’ talent and skill are apparent in the results of his breast revision procedures. The goal of breast revision surgery is to correct any problems caused by the first surgery while ensuring that women get the results that they desire with the least amount of discomfort. Every case is unique and Dr. Ennis will work with you and your unique issues to ensure you get the desired result you are looking for!

What Breast Revision Surgery Addresses

Implants that are too large or small. For women who are unhappy with the size of their breasts after implants and want to replace them with larger or smaller implants, or a different kind of implant (exchanging saline for silicone, for example).

Dr. Ennis will discuss the importance of implant width with each patient and explain the importance of matching the width to the width of the individual’s chest to achieve that ideal result. If the implants are too wide, they can stick out on the side of the chest; if they are too small, there can be a wide gap between them.

Picking the right width is important in choosing the right implant!

Capsular contracture

A complication of breast augmentation, in this condition, the body builds up scar tissue around the implant. This scar tissue tightens and causes the implant to be hard, and may hurt or push the implant out of place, causing asymmetry that can be seen in clothes and bathing suits.

Leaking or ruptured implants. If there is a leak or rupture of one or both implants, breast revision surgery is used to remove and replace the damaged implant(s). This can be obvious in a saline implant, but may show up as a capsular contracture in a silicone implant.

Ruptured Breast Implants Explained by Dr. Ennis

Rippling

Ripples in the implant that can be seen through the skin are fixed by changing the type or placement of the implant through a breast revision procedure. If the patient has saline implants, rippling can be improved by exchanging the implants to silicone, which ripple much less than saline. If the implants are on top of the muscle, changing their placement to under the muscle will likely improve the rippling.

“Double bubble” effect

When implants are placed in a high position on the chest, and the breast tissue and skin sags off of the implants, it results in a separation between the bottom edge of the implant and the bottom of the breast, also known as the “double bubble” effect. This may also happen if the implant tries to drop down behind the native fold of the breast rather than forward into the breast as it should. This creates the appearance of two folds, or a double bubble deformity. Dr. Ennis can correct this complication either with a breast lift or fixing the breast pocket.

Bottoming out

In cases where an implant moves to a lower position than the natural breast and/or nipple, or drops too low, breast revision surgery can raise the implant position so that it lines up behind the nipple for a more natural-looking breast.

Bottoming out can also be associated with implants falling to the side when lying down. Both of these conditions can be corrected by placing sutures in the capsule to hold the implant up in its proper position, a procedure known as a capsulorrhaphy.

Symmastia

Also called the “uni-boob” (or bread loafing) effect, symmastia is a complication of breast augmentation surgery that causes the implants to be positioned too close to one another over the midline of the chest. Oftentimes, the skin can lift off of the breast bone, causing the appearance of one breast across the middle and complete loss of cleavage. Breast revision surgery can correct this by repairing the breast pockets that keep the implants in place and separate from one another.

The surgical method to correct symmastia can be a very complicated repair that may require additional slings and support be placed to hold the implants apart. These can be done using acellular dermis, known as AlloDerm® or Belladerm™, or silk scaffolding to reinforce the tissue. Dr. Ennis is known to be an expert in this area and has repaired many breasts that have this complication. He has even given talks to international groups of surgeons on how to repair complex cases of symmastia.

The Breast Revision Procedure

Breast revision surgery is an outpatient procedure and usually takes between one to three hours to perform, depending on the amount and type of correction needed. The procedure is tailored to each patient and their individual needs, and could include various forms of treatment. In cases where capsular contracture is present (where the breast implant hardens inside the breast pocket), a capsulectomy procedure is performed in order to remove the scar tissue that has formed around the implant.

To correct implants that have “bottomed out” or are falling to the side, excess skin is trimmed from the breast crease and the breast pocket is tightened so that the implant is held in place.

For patients who are experiencing symmastia, the doctor will either reposition the implant so that it is under the chest muscle, or he will suture the breast pocket so that it holds the implant in position. Oftentimes, Dr. Ennis will use acellular dermis, known as AlloDerm® or Belladerm™, or silk scaffolding to reinforce the tissue.

When the “double bubble” effect is present, the doctor can exchange the implants, remove excess breast tissue, adjust the breast pocket or use a combination of these methods.

If the sole purpose of breast revision surgery is to enlarge or reduce the size of the implants, a different size implant can be used to replace the old implants during this procedure. Dr. Ennis is known to be an expert in these areas and has repaired many breasts that have these complications. Breast Revision Recovery

There is typically a small bandage placed under the breast following the breast revision procedure, and the patient will be placed in a strap or bra, depending on how the implants are sitting and what was done during surgery. Immediately after surgery, the patient will need to have someone drive them home and stay with them for the first 48 to 72 hours.

Patients can expect a measure of discomfort, and can use doctor-prescribed pain medication for a more comfortable recovery period. Dr. Ennis does not recommend taking aspirin or ibuprofen for the first week, as these medications may increase the risk of bleeding.

Typically, patients are able to return to work after one to two weeks. They should refrain from exercise and heavy lifting for at least six weeks. Smoking is absolutely prohibited the few weeks before and after surgery, as it will interfere with the healing process.
